Addison’s disease is a rare form of primary adrenal insufficiency that may put your life at risk when not adequately treated. In autoimmune Addison’s disease, immune-mediated destruction of the adrenal cortex reduces production of cortisol and often aldosterone; adrenal androgen production may also decrease. Understanding Addison’s Disease
Autoimmune adrenalitis, the most common cause of Addison’s disease in developed countries, occurs when the immune system mistakenly attacks the adrenal cortex. 21-hydroxylase antibodies are commonly present in autoimmune Addison’s disease and serve as useful markers of the autoimmune process.1 Damage to the adrenal cortex can reduce production of cortisol, aldosterone, and adrenal androgens, disrupting the body’s stress response, blood pressure regulation, fluid and electrolyte balance, and other important functions.
Approximately half of people with autoimmune Addison’s disease also have another autoimmune disorder, reflecting a shared susceptibility to autoimmune disease rather than a failure to normalize or balance adrenal hormone levels. Associated conditions can include autoimmune thyroid disease, type 1 diabetes, vitiligo, pernicious anemia, and others.1
Signs and Symptoms
Symptoms of Addison’s disease often develop gradually and may differ between individuals, which can contribute to delayed recognition or misdiagnosis. One of the most dangerous complications is an adrenal crisis, a life-threatening medical emergency requiring immediate treatment. In some people, an adrenal crisis may be the first recognized presentation of adrenal insufficiency.1
During an adrenal crisis, blood pressure may fall dangerously low, and abnormalities such as low sodium, high potassium, low blood glucose, and severe dehydration may occur. Symptoms can include profound weakness, abdominal or other severe pain, nausea or vomiting, confusion or loss of consciousness, and circulatory collapse.
Allopathic Options
Because Addison’s disease results in inadequate adrenal hormone production, lifelong glucocorticoid replacement is required. Mineralocorticoid replacement is also needed when aldosterone production is deficient.1 Treatment is individualized, and glucocorticoid doses generally need to be increased during illness, injury, surgery, or other significant physical stress.
Regular monitoring helps ensure that hormone replacement is adequate while reducing complications from under- or over-replacement. If an adrenal crisis is suspected, immediate emergency treatment with corticosteroids and intravenous fluids is essential.
Naturopathic Care for Addison’s Disease
Naturopathic care for individuals with Addison’s disease should complement, rather than replace, prescribed hormone replacement therapy. Supportive care may focus on individualized nutrition, lifestyle habits, stress management, correction of nutrient deficiencies when present, and other strategies that support overall health and quality of life. Any supplements or botanical medicines should be carefully reviewed for potential interactions with corticosteroid and mineralocorticoid treatment.

Licorice can inhibit the enzyme 11-beta hydroxysteroid dehydrogenase type 2, which can increase cortisol activity and alter the effects of prescribed corticosteroid replacement.3 Because licorice can also raise blood pressure, lower potassium, and affect fluid balance, its use in someone with Addison’s disease requires careful coordination with the clinician managing their hormone replacement.

It is important to remember that the use of herbs is not for everyone and should be explored with your naturopathic doctor to identify the best remedy for your health and preferences. Licorice root, for instance, can raise your blood pressure. Its intake should be supervised by your healthcare provider. Since risks on blood pressure control are associated with the use of licorice root, you may have to explore another herbal remedy for your health, which should be guided by your naturopathic doctors. Additionally, most herbs should not be taken during pregnancy or lactation.
